Honorarium *

I agree to compensate for the amount on submitted Booking Form.

Deposit *

$250 Deposit (or $500 Deposit for Camp) is required to secure date(s). This nonrefundable amount can be taken out of the stated honorarium. Note: your requested date(s) will not be secured until this form is submitted and the deposit is received.

Travel & Lodging Expenses *

Reimbursement of all expenses including transportation, meals, hotel, etc. while on the way to and from as well as during your event for Speaker and his wife (or another guest if his wife is unavailable). Outstanding travel & meal expenses incurred by Speaker are to be reimbursed by Host within 30 days after event. Speaker will submit receipts to Host.

Cancellation Policy *

If you need to cancel within 30-days of the event, we ask for 50% of stated honorarium still be due to Garrett Wagoner Ministries.

Final Payment *

Remaining honorarium amount will be due at the time of your event date. Host can give check to Speaker. Please notify our office if other arrangements need to be made.
